RES 22R-03-70 Special X - Broward Health AR 22R-03-70 Attachment A DRR 21-SE-019 Broward Health Medical Group Attachment B 21-SE-019 Broward Health Medical Group Affidavit Attachments: Acting City Planner Molly Howson presented the subject special exception application on behalf of Dr. Shanna Rudy Stephen and the Broward Health Medical Group. Dr. Rudy Stephen operated a primary care office, serving the medical needs of all kinds to patients of all ages. The application was for a 2,900-square foot space within two bays at 7539-7541 West Oakland Park Boulevard, Lauderhill; the hours of operation would be Monday through Friday, 8:00 a.m. to 5:00 p.m. Vice Mayor Dunn sought additional information about the applicant’s medical practice, the vision, and the service the facility would provide the community. Mayor Thurston swore in all persons wishing to speak on the subject item. Dr. Rudy Stephen stated the goal of the subject clinic was to treat persons of all ages, providing them with general checkups, wellness exams, doing X-rays, bloodwork, etc., along with urgent care needs Vice Mayor Dunn asked how many employees the applicant anticipated having. Dr. Rudy Stephen replied, for the present, there would be two nurse practitioners, one medical assistant (MA), and she as the physician. Vice Mayor Dunn inquired as to the number of patients they anticipated seeing. Vera, office manager, stated their goal was to see patients ranging from children to seniors, so patients did not have to switch to another doctor as they aged. They accepted a wide variety of insurances, and with their operating hours, they anticipated seeing 80 to 90 patients a day. Commissioner Grant asked if, over time, they would consider adding more doctors. Page 6City of Lauderhill

March 28, 2022City Commission Meeting Meeting Minutes - Final Dr. Rudy Stephen answered yes. Commissioner Grant asked if Broward Health Medical Group was associated with Broward Health, Broward County. Dr. Rudy Stephen stated they were not. Commissioner Grant wished to know more about Dr. Rudy Stephen’s medical background. Dr. Rudy Stephen replied she was born and raised in Plantation, Florida, attending Parkway Middle and Stranahan High School, after which she attended University of Florida, then Meharry Medical School in Nashville, Tennessee, then did her pediatric training at Morehouse School of Medicine in Atlanta, Georgia, returning to South Florida in 2017 after graduating, where she wished to practice. Mayor Thurston opened the discussion to the public. Winnie Mae Cooper, Lauderhill resident and president of the West Ken Lark Homeowners Association (HOA), asked about the exact location of the subject facility. Ms. Howson replied the location was 7539 to 7541 West Oakland Park Boulevard on the north side of Oakland Park Boulevard just west and across the road from the Lauderhill Sports Park. There were several medical offices in that area, but the City code had no distance separation requirement for medical offices, preferring for them to be grouped, thus offering a variety of medical services in a single area. A motion was made by Commissioner L. Martin, seconded by Vice Mayor M. Dunn, that this Resolution be approved. The motion carried by the following vote: Yes: Vice Mayor M. Dunn, Commissioner D. Assistant City Attorney Angel Rosenberg responded all professional services agreements allowed a maximum of five years per bond, as this was the way they were written; they could not exceed five years for bond purposes. As some of the services provided could impact the bond, there was language at the very end of each contract specific to each bond. If issues arose with the service, contracts could be terminated. Vice Mayor Dunn observed it was $12.00 per student, and the City received ten percent of the revenue, while the organization kept 90 percent. Parks & Recreation Director Scott Newton stated they did 80/20, or 90/10 depending on the program and the revenue generated. Vice Mayor Dunn asked if the program would be at no cost to the City. Mr. Newton answered correct. A motion was made by Commissioner L. Martin, seconded by Vice Mayor M. Dunn, that this Resolution be approved. RES 22R-03-62 Title VI Transit Compliance Plan 2022 Yearly Adoption AR 22R-03-62 Annual TITLE VI Compliance Plan March 2022 Attachments: Vice Mayor Dunn referred to page four, asking if the language indicated that, as well as verbal translations in Creole and Spanish, there would be written translations. Assistant Grants Manager Tameka Dorsett answered correct. Vice Mayor Dunn knew the City recently authorized a list of vendors capable of doing such translations. Ms. Dorsett affirmed this to be the case. Vice Mayor Dunn referred to page 11, stating goal three spoke about fighting discrimination in homeownership insurance, and one of the specific remedies proposed was to hold a community forum to review concerns for homeowners’ insurance rates, and their effect on home buying, a very huge issue at present. She asked if City staff had a forum already planned. Ms. Dorsett replied not currently, but they would moving forward. Vice Mayor Dunn wished to discuss the matter with City staff, as she was sure they received numerous calls from residents about homeowners insurance. On page 23, where it mentioned environmental justice in low-income populations, she noticed one of the suggestions was Public Works, the GIS manager, and the City planner would analyze environmental challenges, looking at social determinants of health, economic, and social effects. She wished to know if this would be an ongoing study. Ms. Dorsett answered yes; it would be an ongoing study. There were some changes that were needed to the plan, hence the reason for the subject resolution, so this would be done on an annual basis. Vice Mayor Dunn was pleased to know under the subject plan the study would be done on an ongoing basis, so the City could monitor the health of the community around various areas, noting this was something to be spearheaded by Ms. Dorsett’s department. Ms. Rosenberg added another reason to do the study on an annual basis was there were some federal agencies tied in with the subject initiative, and the Federal Transit Authority and the U.S. Department of Housing and Urban Development (HUD) required a yearly plan update, due to the many policy changes made each year. Vice Mayor Dunn thought she might be thinking of something different, as she expected the City to do a comprehensive assessment, looking at CDC data, the Social Vulnerability Index, etc. Page 13City of Lauderhill

March 28, 2022City Commission Meeting Meeting Minutes - Final Ms. Dorsett replied Vice Mayor Dunn was accurate, as the City would do this through the City’s Affirmatively Furthering Fair Housing (AFFH) Plan, and the Consolidated Plan, so the study would be done on an annual basis, and on a five-year basis. Vice Mayor Dunn remarked this would be helpful when the City sought future partners, funding, etc. She referred to page 24, noting the mandate was for the City to use minority, women-owned, and disadvantaged businesses, asking if when people signed up on the City’s vendor list, were these questions collected, as she was under the impression this information was not collected on the certificate of use (COU) application. City Manager Giles-Smith believed this was something Finance or purchasing staff would do, not the federal program. Vice Mayor Dunn understood; according to the federal program, they mandated the City to identify women-owned, minority-owned, and disadvantaged business enterprises. Ms. Dorsett explained once the bid went out, the City staff collected data on the backend as well; as vendors were listed, staff had a list that documented if a business was a small business, black-owned, minority-owned, or if they were Section 3, as this was a requirement of the program, so the information was carried, just not collected up front. Typically, Purchasing staff had to ask those questions when a business applied to be placed on the vendors registration list, and using a spreadsheet, staff would sort through to see which vendor fell in which category. Vice Mayor Dunn noted, for example, on page 24, item B, it said the City would use affirmative action techniques to facilitate participation by placing qualified small and minority businesses, and women-owned business enterprise on the City’s solicitation list. She asked how staff would go about doing that if they were not collecting the data on the front end. Ms. Dorsett replied the Grants Department only collected such data on the backend. Vice Mayor Dunn thought this was something that should be looked at. Ms. Dorsett commented it did state in the regulations the City had to show its best efforts to ensure the data was being collected. Vice Mayor Dunn wondered if it was the City’s best effort doing it in the current manner. Ms. Dorsett said the information was not being asked up front, as the information in the bid documents indicated no preference, because the City could not show local preference, but it was stated that the City encouraged small businesses to apply for City grants. Vice Mayor Dunn understood, but the subject policy said City staff would place Page 14City of Lauderhill

March 28, 2022City Commission Meeting Meeting Minutes - Final qualified small, minority, and women-owned business enterprises on the City’s solicitation list. Deputy City Manager/Finance Director Kennie Hobbs indicated staff could incorporate collecting the data on the front end into the new process. With the recent roll out of the new small business enterprise (SBE) and local business preferences, the City had to collect additional data, so staff would revisit the City’s application to ensure all the relevant questions were asked of businesses to get the information needed to ensure the City was in compliance with the grant requirements. Martin asked for the resolution to be read into the record. City Attorney Hall read the subject resolution into the record as follows: Whereas, on February 25th, 2022, President Joe Biden nominated Ketanji Brown Jackson to serve as the 116th Associate judge on the United States Supreme Court; and, Whereas, Judge Jackson was born in Washington, D.C. and was raised in Miami, Florida; and, Whereas, Judge Jackson attended Harvard University for college and law school; and, Whereas, Judge Jackson, in her professional career, worked as a public defender; and, Whereas, President Obama nominated Judge Jackson to the U.S. District Court for the District of Columbia in 2012, and she was confirmed with bipartisan support in 2013; and, Whereas, President Biden nominated Judge Jackson to the U.S. Court of Appeals for the District of Columbia, and she was again confirmed with bipartisan support. Now, therefore, be it resolved by the City Commission of the City of Lauderhill, Florida, that the City Commission of the City of Lauderhill strongly supports Judge Ketanji Brown Jackson’s appointment to become the 116th Associate Judge on the United States Supreme Court. XII QUASI-JUDICIAL MATTERS, FIRST READING XIII QUASI-JUDICIAL MATTERS, SECOND READING XIV UNFINISHED BUSINESS XV OLD BUSINESS XVI NEW BUSINESS XVII COMMUNICATIONS FROM PUBLIC OFFICIALS SHALL BEGIN IMMEDIATELY BEFORE ADJOURNMENT City Attorney Hall thanked Parks & Recreation Assistant Director Daphne Dyer for doing a great job raising his children through the City’s Parks & Recreation programs, while she was doing a great job raising her own child. Vice Mayor Dunn mentioned on the morning of March 28, 2022, the Lauderhill Fire Department hosted a very nice meet and greet for the women in their department, so in celebration of Women’s History Month, she wished to publicly thank all the women in the City’s fire services for all they did. On Wednesday, March 30, 2022, at 5:00 p.m., they would go to the Lauderhill Police Department, to which her colleagues were invited. Commissioner S. Martin reminded residents in east Lauderhill within the borders of 441 and Sunrise Boulevard, NW 49th Avenue, and NW 26th Street there would be a community meeting at the Sadkin Center at 6:00 p.m. Unfortunately, the HOA president for the community passed away, and the aim was to revive the HOA, enable community leaders and community engagement, getting residents active in their neighborhoods. Commissioner L. Martin mentioned St. George Park recently received a new, covered basketball court, and a revitalized playground. Using photographs, he showed in the last few weeks, some person(s) vandalized these amenities, showing little respect for the hard work and dedication of the community. He hoped the people in the community saw the vandalism, as they constantly asked the City to replace or upgrade amenities, but this was the type of behavior taking place in Page 17City of Lauderhill

March 28, 2022City Commission Meeting Meeting Minutes - Final City parks. He asked them to share this information with other community members, and speak to youths in the area about such behavior. Someone lit fireworks on the new basketball court and burnt part of the court; he thanked the company for coming back to fix it, as the court had not yet opened for use. He said community leaders and residents needed to encourage residents to do the right thing, so if they saw something, they should say something. Parks & Recreation staff were doing their best to fix what they could, but the community should not have to deal with such behaviors. Commissioner L. Martin mentioned the Lauderhill 55 Plus Walking Program, stating they went out to West Wind Park the previous week, and set up a tent intending to encourage people to sign up to start walking, and they signed up over 30 people in about one to two hours. Hopefully, they could build that coalition of people interested in the Mayor’s exercise program, as such initiatives only enhanced what City’s parks had to offer. He said it was engaging to see the type of conversation they had with residents, and he encouraged his colleagues to put up a tent at various City’s parks, as they would be surprised at the people they would run into. Commissioner S. Martin stated a few months prior his cousin was murdered, encouraging the City’s police to act, as he recognized some of the tags and names in the pictures Commissioner L. Martin showed; they were associated with nefarious persons. Commissioner L. Martin noted the vandalism was already addressed, but staff still had the pictures they could forward to the Police, and Parks & Recreation staff. Commissioner Grant thanked the Parks & Recreation Department staff, her colleagues and all City staff who participated in putting on the Sports Advancing Kids Education (S.A.K.E.) Awards; pictures from the event were still being gathered, so they would be shown at the next meeting. She said the event was well attended and received, and she believed the children, coaches, volunteers, and parents had great fun. The quarterly food truck roll took place with a great turnout, and she thanked everyone for their support, noting only the ice cream food truck did not sell out. On March 31, 2022, the Unity in the Community event would take place, at which staff from the fire and police departments would be out in the community; they typically made it a great success. The last event was in Cypress Hollow, a gated community, and the community really appreciated the effort; they were able to engage with many residents. City Manager Giles-Smith mentioned a special honor was placed on Lauderhill, as the City was proud to announce the nomination of its own Chief of Police Constance Stanley as one of South Florida’s most powerful and influential black business leaders of 2022 by Legacy Magazine. Chief Stanley was nominated for this prestigious award for both her professional and civic achievements; her profile would be included in the Legacy South Florida magazine’s power issue published on April 17, 2022, as an insert in the Sun Sentinel. The MIA Media Group planned to host their award ceremony, where Russell Motley, Associate Publisher and Editor-in-Chief, would present Chief Stanley with a beautiful plaque. She announced on April 2, 2022, 4:00 p.m. to 10:00 p.m. at the Ilene Lieberman Botanical Park, 3801 Inverrary Park, the City would host Jammin’ in the Park, where Michel’le, Doug E. Everyone was invited to bring their lawn chairs, and have a great time; food trucks would be present, and the proceeds of the event would go toward supporting the City’s Fore Life golf program for the Lauderhill youths. Vice Mayor Dunn reminded everyone African dance classes were given at the Sadkin Center, asking Mr. Newton to provide a preview at the next Commission meeting of the fitness classes offered at all City parks. Samuel Wilkerson, Lauderhill resident, stated, on behalf of Broward Estate/St. George, he spoke with CJ about the vandalism at their park, where he went regularly with his grandchildren. He spoke to some parents and teenagers in the community, getting the word out to them, and he would mention the situation at their next HOA meeting. Mayor Thurston said the City would be bringing back the green series; the first session would take place on Thursday, March 31, 2022, and it would be moved to Veterans Park where they would do some gardening. They would build a box, into which participants could grow their plants over the four-event series. He said anyone interested in gardening should come out, as it was free to join, and refreshments would be served at the beginning of the session. The next Jazz Picnic in the Park, which catered to a more mature audience, would take place on Sunday, April 10, 2022, at the Ilene Lieberman Botanical Gardens from 11:00 a.m. to 2:30 p.m. He noted this was a free event to which everyone was invited to bring their lawn chairs and refreshments. His Move with the Mayor initiative was now at two City locations: St. George Park on Mondays and Wednesdays at 8:00 a.m., and at the Sadkin Center on Thursdays at 6:30 p.m. led by Ms. Dyer. In the green series, on April 23, 2022, at 11:00 a.m., there would be a scavenger hunt and Arbor Day giveaway; as Lauderhill was an All-America City, one of the required actions was to honor Arbor Day, and host a related function; the event was free for all ages, and the flyer would be posted on the City’s website for further information. Mayor Thurston mentioned his Move with the Mayor Soca Step would take place on Friday, April 15, 2022, at 7:00 p.m. at the Lauderhill Sports Park; this was a free event led by Coach Kamal from Tailored Fitness.
